I am sure about 12.04.03 and 12.04.04.
I cannot be sure about the previous releases, and I do not plan to re-install
my OS to check it.

It was very rare. Upgrading from 12.04 to 14.04 increased the frequency
of the problem by a great deal. Instead of once in 2-3 weeks it became
once in 2 days.

Happens both with 3.16-rc1 and 3.16-rc2.
Reproduces quickly when playing video. Just left my computer to play a long
list of HD videos for a few hours and that's it.
Another thing to note, and might be of importance, is the fact that I am using
the computer with closed lid and connected to
Alex Shtof, did this problem not occur in a release prior to Trusty?

Well, I provided the information you asked (tried 3.15.1-utopic, which
is very new) and reported that the problem was not fixed. But the bug
still appears incomplete. Do you need any other information? Is there
any other way I can assist in solving this bug?

Alex Shtof, no need to do any further apport-collects at this point as
the information is already previously provided.
Despite this, could you please specify which mainline kernel
specifically you tested (latest is 3.16-rc1)?

3.13.11-03131103
Downloaded it from here:
http://kernel.ubuntu.com/~kernel-ppa/mainline/v3.13.11.3-trusty/
It was recommended in the page to install kernels that fit my distribution. But
now I see that it would be better to test with a newer kernel. Will re-try with
this one and report back:
Tried it with 3.15.1-utopic too (Linux alex-laptop 3.15.1-031501-generic

The whole system froze again, just before my eyes.
This time the file-system was corrupted too and I had to run fsck after reboot.

No, it wasn't. Despite this, I've re-organized, and re-worded the
article anyways to further prevent this misunderstanding.

Well, that was quick. It happens on an upstream kernel too. I am running
