I also tested the upgrade process from Ubuntu 18.04 to Ubuntu 18.10 and
after installing the ubuntu-release-upgrader I was able to upgrade.

On an Ubuntu 18.10 system I installed all available updates with update-
manager and then ran 'do-release-upgrade -d' and was told I needed to
install even more updates (gnome-software) before I could upgrade. I
installed ubuntu-release-upgrader-gtk from -proposed and tried the
process again w/o

To clarify my last comment nautilus and unattended-upgrades both having
a phasing percentage of 0 so won't appear in update-manager but are
upgradable via apt so do-release-upgrade complains about not all updates
being installed.
