Okay.
I reinstalled through synaptic package manager.
This did not resolve the issue with the dialog box buttons.

I tried running the
"firefox -ProfileManager"
from Konsole and although it appeared to accept the command, nothing happened.
Here's what I did after that:
I created a new user, logged out and logged in as that user.
Opened Firefox and tested opening a pdf, dialog box looked the same
Thank you for reporting this to Ubuntu. Sometimes Firefox profiles can
become corrupt and/or extensions can conflict. Could you please try
this with a new profile and see if the problem still exists? You do not
need to delete your old profile to test. If this helps, you might want
