Ah, killing gnome-volume-control-applet from System Monitor gets rid of
it and it doesn't appear to be coming back, but I'll keep an eye out and
update the bug report if it does. Thanks Sebastian!

Also, I should add that running killall gnome-panel doesn't make the
second icon go away.
