In this case, please give me the USB ID as it is after your device is
switched by usb-modeswitch, which device (e.g. /dev/ttyACM0 or whatnot),
along with the logs from /var/log/syslog, so we can see if
modemmanager's udev rules need to be adjusted or if it's an issue
directly in code.

That's right.
For some reason, it works when it feels like.
The fact that work, when altering usb_modeswitch is a large coincidence.
After testing in 3 different machines, it is not usb_modeswitch.
Also, it will always works with wvdial or another pppd dialer. And they
depend on usb_modeswitch.
Raul, so should we take from your comment and update that there is no
bug anymore?
Everything in the description points to usb-modeswitch (these files are
shipped by usb-modeswitch-data). If you have another issue with
NetworkManager, please open a separate bug so we can take a look at it.

I can confirm that the problem is not with usb_modeswitch, but with
networkmanager (or family).

It's strange that this patch should fix any problems. USB_ModeSwitch can only
process three bulk messages in a row so far. The parameter "MessageContent4"
will just be ignored.
The message endpoint is determined automatically, and - since there are
a LOT of different modems that use this config
This is an upstream bug, did you report this to the upstream forum ?
