Some focused windows do not appear on top of unfocused windows,
It also affects the new alt+tab switcher in 11.10, making it appear
underneath other windows.
This time, there wasn't really anything needed to reproduce it; it was
going this way just after I logged in and hasn't quit.

It is very irregular. I thought it had gone away for a time but it
recently recurred, and went away after I closed all open programs. It
seems to only happen if there are a lot of programs open at once, and I
suspend and resume. I can't reproduce it though.

I found out how to reproduce this effect in a different way.
1: Open two instances of program A which has an icon in the Launcher
2: Open one instance of program B. Program B should have focus
3. Right-click the icon for Program A, and click the top selection with the
program name.
