For me this happens when I try to open Pidgin from the top right letter icon.
What happens is that Pidgin kind of starts, but is invisible, the Pidgin icon
is visible with the arrow in the Unity pane. To get Pidgin visible I do:
1) right click the pidgin icon in the Unity pane - click Quit
2)
Thanks for taking time to report and helping to make ubuntu better. Its
been a while since bug was reported, there have been alot of changes in
Unity over the last couple of months. Can you please try the latest ISO
of the Ubuntu 12.04 beta and let us know if the issue have been resolved
for you.

I'm also running the same version of Eclipse (Indigo), also from
Eclipse.org but my build ID is a little older: 20110909-1335 . But I
don't think Eclipse is to blame here.
Sorry, currently I don't have the required hardware to test multimonitor
setups, otherwise I sure would have done that.

problem persist with single display as well?

Eclipse itself is _not_ from the Ubuntu repo, but downloaded and updated via
the Eclipse update mechanism.

As you can see in the video, I am clicking the Eclipse launcher icon,
Eclipse opens the splash but then does *not* open its main windows.
Still, the small arrow is visible aside the launcher icon. I select Quit
from the context menu, Eclipse opens the closing dialog and then closes.

Sure. Just open the app in question. Unity then thinks it is open
(displaying a small arrow besides its icon), but the window is not
