xubuntu-artwork uses an old version of my theme, which doesn't include
the string (I'm not sure as to whether it works well with mountall)
while ubuntustudio-look doesn't seem to support fsck at all. Marking the
task as invalid for both packages.

Jamie, the reason I suspected apparmor was twofold. First I grepped
for dev/shm in /etc, found some likely looking files in
/etc/apparmor.d and then set about purging apparmor to see if it was
the problem.
The results I saw (from fgrep dev/shm -r .) contained:

Luke, I tested your Orca fix.
Before I updated my system, I removed my doed .bash_logout and
/etc/gdm/PostSession/Default file modifications.
When gnome-orca package is updated, I restart my system.
First login operation is works perfect, Orca is talking fine both GDM login
screen and after login
Same for me, headet's shown under "Hardware", but not under "Output" or
"Input". When I kill pulseaudio and start it again manually (either as
shown under https://wiki.ubuntu.com/PulseAudio/Log or by just typing
"pulseaudio" or "nohup pulseaudio &") then the headset is also shown
under "Output" and
