Happens occasionally (maybe once a day on average) and typically after a
reboot... The keyboard setting from the Language Icon in the top right
is set to 'English UK' - there is also an 'English US' option - and
simply re-selecting English UK fixes it
I've verified that x11-utils 7.6+4ubuntu0.2 works as expected in Ubuntu
12.04.1, please push it to precise-updates.

Further testing, apparently to enable unique Tilt packets a certain packet has
to be sent to initialize the mouse properly, evdev is not doing that, but once
I do do that then the mouse sends packets with data of:
