$rcmail_config['create_default_folders'] = true;

The folders are created on login (i find them in dovecot maildir), but

I would think this is a client setting which you cannot
influence. Unless you send HTML mail. Plain text is text without
formatting I'm afraid.

On my system, Dovecot is creating the folders, not Roundcube. But the
rest all happens properly for me. My config is:
