Since my previous posting, we've run dozens of tests trying to work out a
pattern to the failures. The problem appears to be related to the internal
format of the video file. We've tried three different formats, with the
following results:
1. AVI, Motion JPEG codec - no failures

I have OOo Impress installed on two Windows XP (SP2) machines. It's version
2.1 on one, and version 2.0.4 on the other. One machine is a relatively new
laptop, the other is a five-year old desktop. They have different video
chipsets and drivers.
