Well let's see if my setup can help.
1) Use your MTA's lookup function to query exchange. In Exim I use
lookup verify.
2) I use exim's filters to look at the message after SA marks it up to
do this.
3) In exim I just setup a router for all e-mail to be passed to my
exchange server.
4) Why would
I was having similar problems and I finally setup a rule to limit
message sizes that were sent to SA which seems to have resolved it.
Perhaps it's some sort of spammer DoS?

Actually I mentioned that to my Barracuda sales person (we have the
Spyware Firewall which is really good) and he told me that they started
with SpamAssassin, but have since moved to their own software.

Well I use exim with exim-acl enabled. SA scans based on an exim
transport and virus scanning is done during SMTP reception using an ACL.
It has been working well for us with about 20K messages per day reaching
SA (doesn't include viruses sent to /dev/null). We also use an exim
system filter to s

puts in syslog now. It lists each rule hit for a spam as well as the
score.
