This is a hardware problem.
We had the same problem on a V210. Another admin re-installed the OS at
the * stage, and then looked in /var/adm/messages and solaris 10 with
it's new predictive hardware analysis said replace motherboard and
replace memory.

disk from solaris sparc to solaris x86 on opteron.
Our main issue was with support: according to our research, x86 support
is not present in version 5 of vxvm, and please correct me if i'm wrong,

We use HBA's for tape and disk access. It works very well. You do not use DMP
or MPXIO to configure the tape HBA. All you do it install the card, drivers
and then modify st.conf (if using solaris) to find the (tape) luns, just as

space, then remove the 40GB LUN, then shrink the volume and then remove
the temporary space?
Another option maybe is to mirror to other LUNS and then try and shrink
and remove...

with VxVM 5.0 + mp1.
We have a Sun 6540 (Flexline 380) zoned to it over the SAN. There is
only 1 switch in use. 4 ports on the array are zoned to 2 HBA's on the
server, with a 3rd dedicated to tape.

I would try initialising the disk group from scratch again, but don't
mkfs the filesystem. Chances are that the dg info has gone west, but
the filesystem maybe intact.
Also, if the dg has gone, then it won't let you create the dg with the
same name. Then a restore might be needed.

I believe you can run vxdiskadm, option 5 Replace a failed or removed
disk.
The failed disk is sde and the replacement disk is sdf. It been so long
since I've had to use that option, but I think it might also let you
initalise the disk in that same option. Otherwise, optio
ename them, but it doesn't really matter.
