I have no clue how to address your header issue.
Just out of curiosity, did you transfer your settings and data from an older

Hi Esther,
Open settings, general and keyboards and near the bottom of the list of
all choices on that screen you'll find dictation. Once turned on here
the button should appear on all edit fields for you.

I don’t know of any other spot to change the header and footer stuff, but I do
have a thought on the dictation thing. Go into settings/general/keyboard and
enable dictation. See if that is on. Hope this helps,
