There is a product called RE remote. It plugs in to the bottom of
the phone and can control a variety of devices. It cost around $40-$45 from
amazon.com. There was a podcast on applevis about it. It does require
accessible. I should explain

I've been wanting such a thing for the two years I've had an iPhone,
and so far the only thing that comes close, and it's not even really a
true remote control in the sense of transmitting an infrared data
burst, if that's what you're looking for, is the Verizon app suite for
the FiOS set-top box.
