The only two settings as far as ending time I have ever seen is an actual
time, which by default is one hour after the start time.

I just opened Fantastical 2 double tapped on new event.
Did the two finger double tap and dictated the following:
"Dr. Test july 27 at 9 AM alert 30 minutes"
Then I double tapped on the add button.
The event was correctly created with an alert of 30 minutes before.
