This really sounds like the speaker volume is at 0. I know you said you already
tried this, but I would try the volume again, e.g. make sure you are on the
home screen, unplug your headset and do a 2-finger swipe down so Voiceover
starts reading all the apps on your home screen. Now repeatedly
I removed IMO from App-switcher, VO is still through the headphone.
