Um, my apologies if this is a stupid question. I found some info on
iTreadmill, but I'm still not sure if iTreadmill is a pedometer for real
world walks and runs, or something that can be used to monitor activity
from actual use of a treadmill.

It is mostly VO friendly. The only thing is some of the rather strange labeling
of the buttons. However, after you learn how to use it, it works just fine.
It uses the iOS Devices accelerometer. That will monitor the motion of the
iPhone, or iPod Touch.

Yes, and I use it on the treadmill regularly. I also use it on the excersise
bike as well. I don't think the distance is correct cause I haven't calibrated
it for the bike, but it's good enough for comparing times and distances.
