Do you think it is an issue of SqlExtension? Or is it supposed to require
TCP/IP enabled even on the same server?

It was TCP/IP, which was disabled. I played a bit with ports, and it really
turns out that 1433 is required for default instance, but named instance can
use a different one.

locations where it could be, depending on what version of the platform sdk is
installed
