Several reasons why that occurs.. first are u using a SCSI for ur tape??Might be some problems with SCSI communication. Did u check with a ps -ef for any mt processes running.
Generally if any mt command crashes..u might not be able kill process coz the crash may be at the kernel level.Even if u do a kill, a Zombie will exist.So restart system..Rather crude method..but if its a kernel level crash ,u aint have an option Anyways check hardware connections and the modules u have installed.
