i think that isn't a connection problem because the Linux can recognize the SCSI tape. (it is registered in the file /proc/scsi/scsi)

this other than a reboot on the machine.Restarting the
tape drive wont help.
U need to find out why the mt-f command crashed(if the
problem recurs).Are all ur tape modules (aic7xxx,sd st
etc in place).Next check connections
