Okay, I just released version 0.6 of Semantic Google Maps, that has the revised form input that was previously demoed on Discourse DB. It can always be changed at a later point to use something like LocalSearch, although my hunch is still that, at least with LocalSearch's current state, that will present too many problems to be worthwhile.
