I ran that first via the "Initialise or upgrade tables" button on the
Special:SMWAdmin page. It indicated success saying, "Internal properties
initialised successfully."

Taking your suggestion, I also ran SMW_setup.php from command line. That
also indicated success.

Still, however, smwfSetupExtension is the only Extension Function listed.

I'm thinking it might be a PHP configuration property?
