I'm interested in what you are using in your hospital systems to educate 
nursing staff (beyond sepsis recognition/guidelines/bundles) towards guiding 
patients that are surviving sepsis.

Before discharge any specific education plans to those most at risk for sepsis?

At discharge what type of follow up care are you incorporating for sepsis 
survivors after they leave the hospital?
