Our organization worked with local EMS, put together a prompt card and educated the area's 300 paramedics. We started a 'Sepsis Alert' in Dec/12 where the EMS call in prior to arrival at the ED if the patient fits the parameters for severe sepsis/septic shock, and they deliver the first IV bolus in the field prior to arrival. We have had very significant reductions in mortality with this simple intervention based on recognition and fluid resuscitation started in the pre-hospital phase of care.
