Our facility performs concurrent reviews on all potential sepsis patients.
When patients meet the 3 criteria for "severe sepsis" but the physician 
documentation is very clear that "severe sepsis" is not believed to be present, 
would this case be abstracted as YES for severe sepsis? (chances are the case 
would populate for abstraction if  "sepsis" is captured by the medical coders. 
This component of the abstraction is very confusing when there is conflicting 
medical record documentation.
