Kathleen,

Currently the specifications manual does not account for conditions such as 
this, so it would not be considered organ dysfunction for purposes of the 
measure.

Patient with ESRD is admitted with a creatinine of 3.0. This patient normally 
maintains creatinine levels of 2.5. While we would not have used a creatinine 
result of 2.5 for this patient as one of the criteria to determine the presence 
of severe sepsis (as it is normal for this patient), would we use the fact that 
the result has now jumped to 3.0 to meet the organ dysfunction criteria for 
severe sepsis?
