Here is the criteria from the core measure specification manual pertaining to 
severe sepsis presentation time:





Specifications Manual for National Hospital Inpatient Quality Measures 
Discharges 01-01-17 (1Q17) through 12-31-17 (4Q17) 1-216



*         If severe sepsis is present on arrival to the Emergency Department or 
severe sepsis is identified in triage (all three clinical criteria must be met 
or documented during triage), the Severe Sepsis Presentation Time is the time 
the patient was triaged in the Emergency Department. If more than one triage 
time is documented (e.g., "Triage started" and "Triage completed") use the 
later time reflecting triage is completed.



*         For patients who arrive to the ED with severe sepsis clinical 
criteria met or physician/APN/PA documentation of severe sepsis, that bypass 
triage or a triage time is not documented, use the ED arrival time.



In this case, it would be interesting to see the BP on arrival, the WBCs, and 
the suspected source of infection, especially with the lactic acid of 1.6.

If the BIPAP is documented for acute resp. failure at 02:10, would it not mean 
that severe sepsis criteria are met at 02:10??  Then septic shock is documented 
at 02:45 so septic shock onset would be 02:45 unless it met by CMS criteria 
prior to that time???  We are to use whatever time is the earliest.. Correct?
