It could be the read receipts.  I have only seen a read receipt requested
from him once though.  The emails are not going through after the error
occurs.  He is unable to send anything until he either switches to his ISP
or deletes the account in outlook 2003 and recreates it.

Are you sure your emails are not getting through?  I have a similar 
problem and its seemed to revolve around an issue when using MS Outlook 
2003 and James.  It appears that read receipts sent from recipient back 
to sender never actually make it, and instead cause an error similar to 
the one you describe.
