I hadn't thought of putting my phone on/off hold, but my system is also having this problem. I call my cellphone via asterisk from sflphone-client-gnome 0.9.8 on my Ubuntu 10.10 system. I can hear myself dialing. My Cellphone rings, I answer and hear nothing on sflphone the end. I turn hold on/off and I can now hear myself. Another thing that can make it work is to simply hit a digit on the sflphone keypad (like the number "1") and it starts working.**

I'm using the normal version of sflphone from the Ubuntu Repository at the moment, that also seems to mean that if I try to change the "IP Call" under settings, Like to change the local port from 5060 to 5061, it terminates sflphone.*

*I'd try the nightly stuff which usually works pretty well, but evidently there are mixed versions in the nightly repository (snapshot20101029 for sfl-client-gnome and snapshot20101025 for the sflphone-common). I'll try later.
