I thought this might be a firewall problem so I tried a stun server with
the following result:

there are separate streams for ringtone and the calls.
Load up pavucontrol and leave it open on the desktop in the playback tab
Establish a call with one of th echo test sites
(sip:[email protected] <mailto:sip:[email protected]>)
Check the volumes on the playback and recording tabs.
