You know if you take a micrometer to the inside of the shafts your extending 
and find what the I.D. is you will be able to order extensions that fit.

Something I've done is to take a snug section of steel shaft, ala what Tom 
Wishon said, but just cut it long enough to serve as the splice. Half inside 
the shaft, half sticking out. Then use a graphite shaft butt cutoff over the 
protruding steel splice for the extra graphite length.  I save a collection of 
different cutoffs for this purpose, since they vary in ID.
