maybe going out on a limb here but I think this has to do with marketing
and decided who to design the site for, you use feedback and statistics to
make certain types of predictions and assumptions, and certain choices..
and you design the site based on knowledge of the way your visitors will
behave while visiting your site, i.e. if they are more likely to click a
certain link or ad more often in one location on the page than another,
and it's an important link / ad then you put it there, if it's not
important you may put it in the less convenient place.. you also have to
live on facts like the average length of a website visit, which is painful
to us web dev's ;p because it's incredibly short, web surfers are the
worst as far as attention span goes and if you're site isn't incredibly
easy to navigate and nice looking you'll lose most of them
