It depends on:

1.  Site of bite.

2.  Size of spider and amount injected.

3.  Age, size, and health of bitee.

4.  Use, or not, of anti-venom. 

Being bitten in the balls while using a traditional outhouse is not
uncommon.  Good practice sweeps a stick around the hole before sitting down.

Are you sure this was a Black Widow?
I was under the understanding that one bite is lethal to a human. It is a 
neurotoxin paralysis to the nervous system and kills within 15-20 minutes.

Am I mistaken?
