In a regular telephone set, how can you differentiate between an
attended and unattended call transfer?  Usually, the user will press
the 'flash' button, dial a number, and either wait for the user to
pick up the phone, or hang up while he hears ringback.

However, this makes it very difficult to differentiate between
attended and unattended transfer.  If the user hangs up while in
ringback, a CANCEL is supposed to be sent, and then turn it into an
unattended transfer.

Any solutions for this?
