A request can traverse any number of proxies, any of which can record-route.
Each one will add an additional record-route header, so that an INVITE which
reaches the called party will have a "stack" of these headers, with the one
on top being from the proxy closest to the UAS. The bis-03 spec is quite
explicit about record-route processing in proxies and UAs.
