All excellent reasons why figuring out per-minute billing in a proxy is 
quite a difficult proposition.

In a PC->PSTN scenario, the gateways are ideally suited to determine the 
per minute usage, since they know for sure. THis isn't always possible, 
especially in wholesale and prepaid scenarios. There, you need to employ 
B2BUAs plus a whole bunch of security mechanisms to avoid fraud.
