A. all the offer/answer negotiation has to happen within set of requests and associated responses... and can not happen across request messages.
B. according the set of RFCs being published till today, does not specify we can have the scenario where in PRACK without SDP and 200-PRACK with SDP... If some stack behaves like that, it's a protocol violation @ some place in that set of sip entities. Probably "draft-sawada-sipping-sip-offeranswer-01" would give you a better understanding about offer/answer model we can think of could happen.
