A call hold is achieved via an offer/answer exchange. There is nothing 
that says an offer/answer must make only a certain kind of change - many 
kinds of changes may be made at the same time. So the change in 
directionality that is typically associated with going on/off hold may 
be done alone, or may be combined with changing the address/port.
