Some quick answers:

1. The behaviour of a proxy is clearly defined in RFC 3261. There is only
limited freedom on adding, modifying and deleting headers of a message. 
E.g. a proxy must not change the message body

2. If you need more freedom and e.g. touch the message body than you have to
terminate and re-originate. That's what a B2BUA does.

3. A B2BUA is typically used for media transcoding, shielding a network
(security perimeter), actively impact a session (terminate for pre-paid and
lack of credit)...
