You should check out the concept of 'SIP Trapezoid'

The ACK message is probably being sent from the called party back to the
address on the contact HF - which is the NATed IP of the calling UA.
Check the openser nathelper module to fix the contact HF or have to proxy
add a record-route so subsequent requests will be sent to the proxy and not
directly between the parties.
