Maybe the case is assumed not to be a problem (and hence not addressed/specified anywhere) because if the media gateway is converting from inband to rfc2833 you would assume that the downstream connection is only expecting rfc2833 and therefore not even trying to detect dtmf. In other words inband leakage shouldn't be a problem.
But that only covers the next leg of the call.
