Click to Dial is only one aspect of 3pcc.  A 3pcc'er can do all sorts of good 
and bad things associated with the call.

I'm not saying 3pcc can't be done using REFER.  However as I indicated, one of 
the goals of rfc3725 was to use basic sip instead of requiring exentions such 
as REFER.  The are a few of the the drafts discussing call control using SIP 
extensions.
