Adding supported timer is required if including refresher=uac.

Adding supported timer is not required if included no refresher or included 
refresher=uas.  This simulates a proxy interaction.

It is abnormal behavior (as indicated within RFC 4028 Table 2); thus it can 
basically act however it desires (assuming RFC 4028 doesn't explicitly define 
how to handle the abnormal situation).
