But in giving the rules for proxies, we must take care of the case
where a downstream proxy sends a 199.  This is where we get the rule
that if a proxy propagates a 199 for an early dialog, then it should
not send a 199 for that early dialog itself.  Once we've set that rule
for proxies, then a UAS can also send a 199 without breaking anything.

But it does look like there is no gain from sending a 199 unless the
corresponding final response will not be sent immediately.
