Out of the blue answer:

The "user" has to have a registered phone on the system, and logged into his
personal user portal (Thus by defintion he is a user with a password).  His
phone will not be on the other side of an ITSP, and is by defintion an 
"internal"
call (regardless of the physical location of the phone--it registered--it is
"internal").

In any case, if his phone doesn't support REFER, then click-to-dial will
not work for him.  Life sucks sometimes, and phones have to support things
for other things to work.
