I don't see why ITSPs will be an issue for click-to-dial. The phone that is sent the REFER is the "local" one. It cannot be on the far side of an ITSP. We are not just sending click-to-dial requests to random phone numbers (well, I hope not...), only to the extensions of logged in users.
