The UI looks pretty good.

On the CSR generation screen:

I think we don't really need to make the server name a modifiable field,
since by definition it's the same as the host you're using to generate
it.

I'd label the Email Address field as "Administrator Email Address" to be
clear about what it's for.

It might be better to put the generated CSR file on another screen from
the input parameters, and make that screen a tab of its own.  This is
because the CSR is paired with a key file - the two must be used
together, so if the administrator were to generate one CSR and send it
off to be signed by the CA (this can sometimes take a day or two to get
results back due to administrative validation steps at the CA), and then
accidentally generate another CSR, the certificate eventually returned
by the CA would not be usable.
