That should work; although it is mostly used if you have 2 gateways and 
only one of them requires extra prefix.
In other cases it's better to do it in dial rule. You do not even have to 
use custom dial rules: all long distance rules allow specifying 'add 
prefix' (passed to the gateway) and 'drop prefix' (removed from the dialed 
digits). Both of these prefixes can be optional, which can be set if you 
click on 'advanced' link.

Post your fallbackrules.xml.in - someone will be able to tell you what's 
wrong based on that.
