If the Mediatrix were able to add the leading digits to CLID it would have to be done based on the CLID's NPI/TON. For example in the UK if the CLID is E.164/National it would need to add a leading 0, if the CLID is E.164/International it would need to add a leading 00, if the CLID is Unknown/Unknown is would add nothing. The only real issue I have with it is that incoming calls are missing a leading 0 on their caller id. As in the number 0412798593 will come through as 412798593. While this might be strictly correct as the number (and the mediatrix is just passing on what it has been given) it is effectively useless since it cannot be dialed and won't match anyone's address book entries (on the old PSTN service they were provided with the 0). Is there any functionality within SipX to add the 0 to any incoming calls from this gateway?
